Employers are provided with the following material, emailed
as PDF attachments (so documents can be printed as required):
- Agreement and Administration Manual
- Employee Benefit Booklets for distribution
- Health Spending Account Claims Forms
The Assureflex Corporation HSA will perform the following
functions (as the Assureflex Plan Trustees and Administrators):
- Review Claims for eligibility and calendar year Maximums
- Provide reimbursement to Employees or payment to Providers
- Provide monthly "Summary of Claims" Statements
Consultation
Assureflex Consultants are available to respond, by email or telephone,
to any questions in regard to Assureflex Health Spending Accounts.
